Extensor carpi radialis brevis

What it is
A shorter muscle lying underneath extensor carpi radialis longus, arising from the common extensor tendon on the outer bump of the elbow and running to the base of the middle finger's metacarpal.
Its origin sits deep within the common extensor tendon, in a position where it is compressed against the underlying bone as the elbow moves through range. That anatomical detail is why it appears so consistently in accounts of lateral elbow pain.
What it does
Extends the wrist and deviates it slightly towards the thumb. Because it inserts on the middle metacarpal, its pull is closer to the centre of the hand than its longer partner's, making it a more purely extending muscle.
It stabilises the wrist during gripping, alongside the other extensors.
What loads it
Gripping, carrying, and resisted wrist extension.
What is commonly got wrong about it
"Tennis elbow" as an inflammation to be rested. Lateral epicondylalgia is most often attributed to this tendon. Two things about the popular account are wrong: the condition is not primarily inflammatory, despite the "-itis" in the older name; and complete rest is not the consensus management — loading programmes are generally favoured. What that means for a specific painful elbow is a clinician's call, and this site does not give rehabilitation advice.
That most sufferers play tennis. The large majority do not.
That it is interchangeable with the longus. Different origin, different insertion, different clinical significance.
