What it is

The deepest layer of the back, lying beneath multifidus. Each is a very short bundle spanning one or two vertebral segments, running from a transverse process up to the vertebra immediately above.

They are best developed in the thoracic region and are small or absent elsewhere.

They are the shortest muscles of the spine and among the shortest in the body.

What it does

Nominally they rotate the spine, which is what the name says. In practice their lever arms are so short that the amount of rotation they could produce is very small.

The more interesting proposal is that they are primarily sensory. They contain an exceptionally high density of muscle spindles — the receptors reporting muscle length — relative to their size, higher than most muscles in the body. That has led to the reasonable suggestion that their main job is monitoring the position of one vertebra relative to the next and feeding that information to the nervous system, rather than producing movement.

This is a well-supported observation about spindle density and a plausible but not settled conclusion about function.

What loads it

Nothing selectively. They work with every spinal movement and cannot be isolated by anything.

What is commonly got wrong about it

That they are trainable. They are millimetres long, buried beneath two other muscle layers, and produce negligible force. There is no exercise for them.

That "deep spinal muscle activation" is a coherent training goal. The deeper you go in the back, the less voluntary access you have, and the more likely a claimed exercise is targeting something you cannot verify.

That high spindle density proves a sensory function. It is strong circumstantial evidence and it is not proof.
